Site has been traced to AKQA, an ad company. The same one that did the Skyrim and Fallout 3 promotional work. Looks pretty clear to me.

Did you watch? It (appears to) apply damage twice. Randomly at that. If it was always this way, as many other games are set up with instant headshot kills, people wouldn't have a problem with it.
