To rewrite Fallout 4, you have to start by rewriting the source code. Otherwise you're just trying to mod Call of Duty into Fallout.

What happens if you get a quest from NPC1 to kill NPC2, and in the middle of the quest the game updates, and you moved NPC2 to another location, or removed NPC1, or changed the quest consequences, etc? How do you handle savegames in an open-world game that is constantly updating?

I've yet to see any proof that Bethesda aren't dense or inept. It's just that their ineptitude matches the quality of the product they're shooting for, aimed at the lowest common denominator. A good giveaway of their competence level is that their buggy games are usually fixed by fans.
