Calculating degree of scatter as you suggest does seem to fit, and could be applied on top of the V.A.T.S. roll, looking again at the hitting the chest comment, so they could be integrated.

Which is why I am still unclear on the details of the real time accuracy mechanics, and keep going on about wobble etc. I originally assumed that there would be a consistent system, but these comments suggest that there are two parallel systems if I am not mistaken.
