If we're talking about Bethesda's way of storytelling I think they would design an entire territory, fill it with unique enemies like the skin-mask wearing Ghouls, then just make them all completely unapproachable as they attack you on sight. At least, that's what Fallout 4 apparently does.

You can blame Bethesda for that. I'm just saying that it is (in concept) a skin mask, and would be a cool thing for Ghouls to be wearing convincing themselves that they look like their former selves. I'm actually surprised we haven't encountered more ghouls with that mentality actually.

Eh, the Children of Atom are so under-developed I think it's fair to make a ghoul cult that sacrifices humans. What might be cool, is a group of ghouls who are sacrificing humans because they hope to become "normal" again. Like the Silence of the Lambs skin body-suit or something like that...

I would say the Legion are under-represented rather than underdeveloped. Their history and details are all there, it's just that there isn't much Legion content in-game. Plus the terrible combat and engine doesn't do their guerilla and melee-based tactics justice.
