Good graphics don't make immersion on their own, but poor graphics can break the immersion that other elements build up. Some moments in Fallout 3 have made me groan... very, very loudly.

Difficulty in Bethesda's RPGs since Morrowind are, as Texas Renegade clarified, simply based on changes to the "die rolls". Also increased XP at higher difficulties kind of makes sense to me, otherwise you'd ALWAYS be the underdog.

Yes but I believe the issue with level scaling is if you come back at level 20 and there's Satan Himself standing there looking slightly miffed, ready to wail into you. That causes a slight disparity.
