First time out of the vault
Character Creation: the number of repair kits in the game implies you were suposed to choose your race as the main character uniquely in the series. Because why else you'd have access to so many kits if you couldn't be a robot by design?

Worldmap: As much as we hate how it ended, you likely was supposed to be independent chosing your mission if you give a thought to the outdoorsman perks. Not sure how'd that work. Maybe you'd have been able to locate the special encounter hexes?

Brahmin Wood: I think originaly you'd get the same BoS-rank if hostages died. Thisis implied by the debriefing-text:
"To show their respect, they renamed their Brahmin stud after you. The Brotherhood, however, will look favorably on you for your next review."
This theory gets strengthened by my my view on other mission where I think originaly moraldilemmas would have been implemented.

Freeport: it very much as it is.

Rock Falls: Nanuk should have been mentioned in the debriefing, especially if you let Ripley released.

Macomb: You evidently were supposed to avoid civilian causalities. Alternate ending would put them into labour cmaps instead of hands of the extremination squad.
Also, a lot of passways seems to be blocked after they got designed.
Design flaw: fromthe start you can see a building (available to go there after gate #3) whichhas a raider on the top. Only,you can't climb up.

Peoria: a new type of moral dilemma could take place here: you either not cause any fatalities amongst the tribe by switching on and off the turrets, OR you let them all die and the broitherhood take over the vault.
Theory #2: Somehow some Vault 0 Citizens ended up here. I have no explanation for the momnent. Maybe the Guardian of Forever took part of this? Maybe Vault 0 had Skynet with a teleportation-device? Maybe vaults were interconnected with teleportation technology? Ok, this got too far-fetched.

Quincy: if RPG-elements would be added, here it could be done on serious level. Like how about extreminating hthe racist human population and giving the town to the ghouls? This is not a gaming theory as nothing implies this, but a fanciftion of course.
But here is a theory: one time I accidently shot a prostitute. Babs never shown up on the recruit-list. Is she an ex-working-girl?
Also:youdon't have proper time tochitchat with the ghoul leader before the beastmen attack (design flaw).

Mardin: seems design-idea was, that you either free the humans and kill the Mother, OR you feed the humans to the deathclaws for deathclaw recruits. Yuck!

Springfield: I think this mission got redone to Macomb.

St. Louis: Barnaky got captured because I stole his ammo back in the bunker...
Also, you definitely should be scolded if you entered the mutant HQ.
And a design-flow: the grenadier on the sniper nest in the mutant HQ is unreachable.

Jefferson: seems more of a bad design, but you can't pass the subway with the cartpo enter the factory.
The door from the radioactive tunnel also is unlockable which seems to be a cancelled content.
Not to mention the supermutants on the side. They should have marched out ata point. At least if you start rampaging the lab, or sg.

Kansas City: evidently ran out of time here with programming. You should have preparation-time to mine the entrances, speak with the major etc.
Also, at the south gate, on top of the nearest house with sandbags on the roof is a supermutant. Youcan't climb up there, which is a design-flaw.

Osceola: I strongly think the most logical way inside the complex would be the tunnelfrom the right. But in reality that works the least.

Junction City: the unreachable floor with the Evil Brahmin demands an explanation!

Great Bend: I think this WAS intended to be a factory, AND destroying the pacification-bot should have been disable all the robots, especially on later missions.

Cold Water: yep, the entire top floor is unreachable. Aside from that, and the really badplaced rocketeer at the final boss, you evidently would be in a side A or side B scenario, pretty much like in Junktown of F1.

Newton: Unfortunate cut content, that Glenda (or Glen?) never shows up at any BoS-bunker.
