Interesting. It would be cool to get energy weapons instead of melee if you're like me and usually avoid them (they look rather like toys i guess) to experience different combat. Goes well with science ( craft bullets etc).
You can also make him gay ( confirmed bachelor ) as ceasar's legion are cool with that if you're male and it goes well with that genius scientist theme.
You could also take the four eyes trait, just because he's a doctor and everything.
With this build you'd have an interesting character because while you'd be the opposite of CL ( medicine instead of survival, energy instead of melee, intelligence and charisma instead of strengh and endurance ) you'd also be intelligent enough to understand ceasar's plan and how it could work out for the best even if it doesn't feel right to you, because after all you don't chose with your heart but with your brains.
In the end, it will be (science and) medicine and not power that will save ceasar and his legion ( hegelian dialectics i guess)
Now, about the changes that you're character must overcome in order to support a fascist dictator, here are some ideas.
Based on his intelligence, he might see- much to his surprise- that the NCR is ineffective and is not helping people and this will torment him to find out why.
He also has to become cynical and disapointed up to a point, because -let's face it- one must have some issues to support the crushing of the weak, no less. So maybe he should start feeling a bit sick of the world around him and at how no one cares about each other and he should come to the conclusion that in the end all that matters is raw power (which he does not pocess but admires and accepts as a necessity) and through it, unity.
It will be more difficult to explain how he would defy mr house- after all his plan also makes sense and because your character is focused on logic he should find flaws in it in order to stand against him.
I can only think of one flaw- mr house relied too much on Benny and now relies too much on the courier, like he's certain that there's no way the courier would not obey him. In that he's almost delusional and he really should have learned from benny, but the fact is he's rather hopeless without him and is probably in denial. Either that or he thinks that his best shot at getting the courier to follow him is being bossy. Well he's wrong and he goes down, have your character kill mr house, outsmart and outgun him, and experience for once the raw power that he's come to support.
About yes man, too many things could go wrong and your character doesn't really gamble unless the odds are in his favor. So even though he'll be briefly tempted by his power, he'll disregard the thought as too risky.
About women and CL misogyny, there's another problem that can only be solved by treating your character's political views as a result of his depression i guess. Because he was follower and they're totally feminist people, and it will be hard to explain why he's suddenly ok with how CL is treating women. So, his reasoning will probably be, again, that women are weak and must inevitably be crushed by the strong. Either that or have him disagree with CL's sexism but hope that when they get vegas and become civilized, history will take its course and in time women will be considered equals again.
About forced labor, he can think the same, either that it's a necessity or that it will fade out after the new synthesis.
