Sonny, I Watched the Vault Bein' Built!

I'm currently on my first playthrough, around level 19-20. I haven't done that many side-quests and I haven't even entered The Strip yet. so far levelling up has been too easy and I'm a bit disappointed since Josh Sawyer had stated earlier that reaching level 30 would be a massive accomplishment that required a lot of playing.
the fast level advancement would be fine however, if the game would offer challenges in another form. wild life can still be challenging and there have been a couple of quests I've had to put off for later levels, which is a great improvement from FO3. but all-in-all, I don't have any trouble finding ammo, stimpaks and other resources. I tagged Survival and have increased it successively every level, but I haven't had any real use for it whatsovever. and I purposefully didn't put a lot of skill points into Repair since I thought it'd offer a challenge in the form of having a hard time keeping weapons and armor in shape. but with the huge amount of weapons you find it hasn't really been a problem at all. now I've boosted Repair all the way up to 90 just so I could get the Jury Rigging perk to be able to repair some unique equipment.
enemies die easily enough - even ones with a high damage threshold go down relatively fast as long as I keep my companions around. I haven't had to waste any special ammo so far. on the contrary, most enemies can't even put a scratch on me because of my high DT. in combat, the main challenge is keeping my companions alive. the final quest in Camp Forlorn Hope was a real disappointment, seeing as how the npc's killed off most of the guards (the few that were there) by themselves, and the "boss" went down with a couple of headshots. I didn't lose a single point of health during that mission.
I guess this is mostly a thread just for whining, but I do have a question to ask as well: have Obsidian changed how the engine handles the difficulty setting at all? I'm not much for changing the difficulty during a playthrough (I'm playing on normal btw), and I certainly don't want to do it if higher difficulties only mean enemies with more HP. what's your experiences with higher difficulties? and are there any mods out there yet that will make the game more challenging?
