In my game I saw him sprinting north away from town, and then he eventually warped back to near town I believe. Seemed mildly bugged.

He is usually to the left as you cross the bridge over from the NCR side of town.

It Wandered In From the Wastes
I had this bug as well. I found Hayes at [spoiler:656d486daa]Camp Forlorn Hope[/spoiler:656d486daa] but he didn't have anything to say about the quest. Sgt. Lee, for some reason, wandered into the wasteland. I got the message that he had died like 6-8 hours after I left Primm.
Also, the radio message for the quest said that the assault had failed, which was also weird.

so "working as intended" as always.
The reason i wanted to know is bc i think the pipboy flashed in top left that i had just completed 1/5 of some quest line and/or area quest. Maybe i will *spoiler* side with the prisoners and see where that quest chain leaves me.
